Overview
Atypical opioid analgesics provide analgesia through a combination of weak opioid receptor agonism and monoamine reuptake inhibition, distinguishing them mechanistically from classical opioids. Tramadol is the primary agent, indicated for moderate to moderately severe pain.
How do atypical opioid analgesics work?
Tramadol and its active metabolite O-desmethyltramadol (M1) produce analgesia via two complementary mechanisms: (1) weak mu-opioid receptor agonism (primarily via M1) and (2) inhibition of norepinephrine and serotonin reuptake, which activates descending pain-inhibitory pathways. The dual mechanism results in additive analgesia at lower opioid doses, though tramadol retains opioid-class risks including dependence, abuse potential, and—combined with serotonergic agents—risk of serotonin syndrome.
